For patients who are linked with a Catholic Medical Partners Primary Care Physician, such as Inspired Health Group care providers, a social work self referral program is now available to support patients in their health & wellbeing.

Although we’re always here to help, we get it - it can be difficult sometimes to express feeling overwhelmed with some factors that influence your health and wellbeing one-on-one with your care provider. These factors may include financial issues, housing, transportation, health insurance, abuse, needing a higher level of care, substance use, domestic violence, home safety, lack of support, mental health, and trouble affording medication.
For patients who are linked with a Catholic Medical Partners Primary Care Physician, such as Inspired Health Group care providers, an online social work self-referral program is now available from Catholic Medical Partners (CMP)! Simply fill out the online form to be connected with a CMP Social Worker who can provide you or your child access to helpful resources.

Your patient portal is also a great resources in accessing support for topics that may not be easy to talk about during a one-on-one visit with your care provider. Messages sent in your patient portal are safe and secure. The MEDENT Patient Portal uses HTTPS to provide encrypted communication between you and Inspired Health Group (IHG). We take great care to make sure your health information is kept private.
