No-Show Fees & Cancellations
We understand things come up. As a friendly reminder, missing an appointment or arriving more than 15 minutes late may result in a no-show fee up to $100. To avoid this, please call us or reschedule at least 24 hours in advance.
What to bring to your appointment
Essential items to bring to your appointment include,
- Insurance Cards: It is critical that you bring your most current insurance card to every appointment to verify your coverage
- Photo ID: Driver’s license or other government-issued ID
- Payment Method: Money to cover the cost of a copay or payment toward any outstanding balances
- List of Current Medications: This includes prescription drugs, over the counter meds, vitamins and herbal remedies. Please also ensure you know the dosages and frequency. You can bring in the actual bottles if that's easier.
Please also consider
- Medical History: A list of past illnesses, surgeries, or ongoing conditions is essential at your first visit with us. At the time of your visit, you can share any changes to your medical history with your care provider. This can also include any vaccination information you may have received elsewhere, like a flu shot. You can also update your medical history anytime within your patient portal.
- Family History: Information on hereditary health conditions is helpful at your first visit. You can inform your doctor of any changes to your family history at the time of your visit, or update your family history in your patient portal.
- Contact Information: Names and phone numbers of any other doctors or specialists you may see.
- List of questions for your care provider: To ensure we cover all of your questions and concerns during your visit, coming prepared with a list of concerns, symptoms, or treatment options is helpful to direct the conversation with your care provider.
- Symptom Journal: If you are experiencing an on-going condition or new medical concern, a journal with a detailed list of symptoms, including when they started, frequency, and severity can help inform your care team of your health & wellbeing.
Appointment Arrival Time
To best serve you, please arrive 10-15 minutes prior to your appointment time. Arriving early allows our patient services team time to get you checked in to your appointment, answer and questions you may have, and go over any paperwork you may need to fill out prior to your appointment.

Payments
Please bring a form of payment to your appointment. Payment is expected at the time of your visit, or before if you have a balance.
